Position: Support Coordinator

Support Coordinator responsibilities:
Support Coordinator perform tasks necessary to assist individuals with achieving the best outcomes, as identified by the individual. These duties usually involve working with individuals directly and often working with the individuals’ support team: family members, close friends, health care workers, and so forth. Responsibilities vary depending on the individual’s needs, but may include:
- Assessing individuals’ physical and mental wellness, needs, preferences and abilities and using these to develop person-centered support plan
- Working with individuals, family and friend support networks and health care professionals to develop support plans
- Listening to individuals’ concerns and providing support and/or intervention as required
- Recording individuals’ progress including documenting referrals, home visits and other contact interactions
- Evaluating individuals’ progress periodically on achieving goals and adjusting goals as needed to improve outcomes
- Monitoring provider services and ensuring individuals are satisfied with services
Mandatory Job Requirements:
- Bachelor’s degree in the human service field (Social Work, Psychology, Sociology, Education, Counseling, Rehabilitation, Nursing, etc.)
- Minimum of at least one year of related experience in the field serving persons with developmental disabilities or related disabilities in a case management or coordination role
- Computer skills with proficiency in Microsoft word to include entering, updating, and maintaining Consumer Information System Strong verbal and written communication skills
- Must possess a valid driver’s license
- Must be able to provide proof of auto insurance and have access to an automobile
