Job Description
The Registered Nurse (RN) in Home Health provides comprehensive nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Your primary focus will be on coordinating total patient care plans with other healthcare professionals involved in care and ensuring continuity of patient care through effective communication and collaboration.
- You will make initial and comprehensive nursing evaluation visits to determine the primary focus of care and develop an individualized plan of care within state-specific guidelines.
- You will perform patient comprehensive assessments at designated time points and adapt the plan of care accordingly.
