You know what connected feels like when you are part of our team. #WhatConnectsYou Job Description Job Summary : The role of Hospitalist Patient Navigator will provide administrative assistance to the Lead Hospitalist APP position through the coordination of the daily work flow of the hospitalist department. This position will utilize multiple systems and modes of communication to ensure patients are cared for in an expedited, efficient and safe manner. Census Management: Publish and distribute patient assignments/rounding list. Ensure accuracy of physician coverage of rounding lists and on call schedule – make changes as appropriate daily. Accurately enter data into the medical record when appropriate as well as census management software. Re-distribute patient assignments as needed when attending availability changes. Answer calls from ED providers and collect patient and clinical information to determine appropriateness of hospitalist admission through use of electronic medical records as needed. Evaluate if patient is patient at FMRC, and if so redirect ER physician to contact FMRC. Re-direct respiratory admissions to their outpatient Pulmonologist as appropriate. Assign patient to initial attending physician team if re-admitted within 30 days to hospitalist service. In accordance with fair and equitable distribution policy, notify next attending physician of patient information and ER provider needing call back in order to accept patient for admission and expedite assignment of appropriate room via Patient Logistics Center (PLC). Assign patients to Advanced practice providers in accordance with fair and equitable distribution policy. Addition of patients accepted for admission through transfer or direct admission to physician distribution list. Upon arrival of anticipated patient to assigned room, notification of the next attending physician in a timely manner and change attending physician in the medical record. Ensure patient assignment to physician coming on shift when accepted by previous physician. Continuously monitor Teletracking and hospitalist census to confirm patient arrival to appropriate unit and under assigned attending physician – change as needed. Administrative: Responsible for accurate distribution of forms (Home Health, DME orders, Rehab evaluations, Hospice forms, Hospital Indemnity claims), to appropriate physician for signature and ensure returned in timely manner. Respond to all pages received on hospitalist pager in timely manner with utmost professionalism. Document and return calls from former patients and their family members within scope of practice, delegating responsibility to appropriate provider and assisting as needed. Collects documents from fax machine, mail, tube system and verifies who is to receive and respond to document to guarantee appropriate and timely response. Assists calls from outside pharmacists concerning patient needs and can take detailed and accurate message to relay to appropriate provider. Maintenance of patient census and attending rounding lists in Ingenious Med. Update provider board daily with accurate list of providers present, census and on call physicians. Refers problems within the hospitalist department to appropriate chain of command. Job Requirements: Associate’s degree in healthcare related field or willing to accept 3 years’ clinical experience beyond minimum requirement in lieu of Associate’s. Minimum 2 years’ patient care experience required. Demonstrates leadership, interpersonal, critical thinking and problem-solving skills. Ability to manage situations related to family and visitors, patients of varied ages and needs, physicians and other departments Proficient in use of computers Ability to multi-task Must possess outstanding verbal, non-verbal, and written communication skills. Must possess superior listening and negotiating skills and be able to handle situations with confidentiality, trust, and integrity. Must possess understanding of cultural diversity and multi-generational complexity issues Knowledge of hospital and overall NMHS policies.
