Case Management: Provide case management to patients seen at North MS Medical Clinics. Liaison Act as a liaison between physician, patient, families and other office staff. Actively promote a patient-focused approach to care delivery. Triage: Triage walk-in patients and telephone calls. Documentation: Responsible for maintaining written or computer record of patient care events performed by the incumbent (e.g.: medications, treatments, procedures). Education Initiate, perform, and develop education materials. Provide and document patient teaching.
