* Lead for CCP Value Reporting– Serves as primary contact, follows up on outstanding items– leads and supports development/evolution value based reporting with payers, communicates with department leaders regarding status and follow-up, Produces and provides reports for leadership * LeadsPayer Platform Relationship– Serves as contact, liaison with Practice Assist, Data Link and Forvis for database entry and storage issues, runs and distributes reports and as requested by department leaders, , assesses patient needs and directs issues/concerns to appropriate care team members for outreach provides patient and caregiver follow-up data, Maintains historical database and tracks trends/patterns. Assembles data and produces reports for leadership. * Leads Quality Reporting for Physician Compensation- provides oversight and needed reporting to support quality component of physician compensation for clinic system * Effectively communicates with providers, patients, and multi-disciplinary care team members via reporting, care gap closure and educational presentations * Projects a caring, friendly, knowledgeable and friendly image to providers and care team members. Displays genuine compassion for providers and care team members, providing excellent customer service Job Knowledge: * Associate degree required * Strong communication (verbal and written) skills required Strong analytical skills required
