**JOB SUMMARY** The Program Coordinator at North Mississippi Health Services is responsible for supervising clinical staff to ensure the delivery of quality care, assessing service utilization, monitoring patient outcomes, and promoting timely care provision. This role involves reviewing patient records, collaborating with healthcare providers, and serving as a liaison with physicians and community resources. **JOB** **FUNCTIONS** Clinical Practice: * Supervise clinical staff to ensure delivery of quality care based on evidence-based best practice guidelines and individualized care plans; staff competency; performance of responsibilities efficiently and effectively and in a professional and caring manner. * Assess utilization of services, monitor patient outcomes, including observation of care and intervene as necessary to assure appropriate utilization of services and optimal outcomes. * Intervene as indicated to promote timely provision of care and utilization of appropriate resources and assure optimal outcomes. * Monitor data collection and ensure operational statistical benchmark goals are met/exceeded regarding: CMW and LUPA rate, utilization metrics for all disciplines, and productivity. * Conference with multi-disciplinary healthcare teams for plan of care development and management and assures documentation. * Assist with care protocol development, implementation and follow-up process for specific patient populations based on evidence-based best practice guidelines. * Assess social and emotional factors relating to and affecting health status of patients and make appropriate referrals to multi-disciplinary teams. * Make patient visits as appropriate.
