About the Role: Youth Crisis Resolution Team Bachelor Mental Health Counselor:
The Youth and Family Intensive Services program consists of several programs that support youth who are at risk of hospitalizations, legal involvement, expulsion from school for a variety of reasons, at risk out of placement, and can benefit from the additional support of individual therapy, group therapy, peer support, and case management services to increase skills, strengthen bonds and reduce the need for outside stakeholder interventions.
This groundbreaking program provides home-based support to youth at high risk of an opioid overdose. This position engages a multidisciplinary team, comprised of Medication prescribers, Mental Health Clinicians, Case Managers, and Peer Support Specialists, and offers high collaboration with School Districts, Health Departments, and other community partners. This program will provide support to individuals and families by linking them with other programs around the youth continuum and with our medical team.
Key Responsibilities:
- Ensure compliance with utilization, quality improvement, and corporate policies and standards.
- Provide direct services to clients as assigned by your supervisor, including assessments, goal setting, and treatment for mental health challenges.
- Conduct screenings, intakes, and assessments within established timelines, utilizing the required tools and formats.
- Develop and update strength-based treatment plans and continuing care plans, ensuring timely referrals and discharge summaries.
- Assess clients diagnoses and mental status and create individualized treatment plans to meet their needs.
- Provide crisis intervention and de-escalation in various settings and assess risk to ensure client safety.
- Collaborate with clinicians, law enforcement, external providers, and psychiatrists to deliver coordinated care.
- Work with clients on safety planning, symptom management, and risk education to enhance support systems and improve overall well-being.
- Utilize therapeutic interventions and behavioral skills training to help clients develop coping strategies, make informed decisions, and solve daily life challenges.
- Support and implement evidence-based practices to ensure the effectiveness of client interventions. Provide case management and connect clients to internal and external resources to support treatment goals.
Qualifications:
- Bachelors degree in psychology, social work, or a related field.
- Valid drivers license and a driving record that meets insurability requirements.
- Successful competition of background, fingerprinting, and drug testing
- Bilingual (English/Spanish) skills are a plus.
Experience:
No prior experience is required, though its preferred.
