Qualifications
About the Role: Adult Outpatient Program: Case Manager
North Range Behavioral Health believes no one should fight behavioral health challenges alone. As a Case Manager with the Adult Outpatient Program, you can expect to collaborate with clinical teams to secure and provide resources to clients with mild to severe mental health challenges and/or substance use disorders. Case Managers work directly with clients across the lifespan to provide compassionate and comprehensive bridge care while clients continue services. These services can include education and skill development, to help clients move towards independence,
Key Responsibilities:
- Conduct assessments, develop safety plans, and provide mental health or substance use counseling.
- De-escalate crises with clients within the community, including those at risk of self-harm, substance abuse, or homelessness.
- Collaborate with law enforcement and community partners to ensure seamless care.
- Provide ongoing support, helping clients access resources like housing, medical care, and social services.
- Document services and ensure compliance with agency standards for billing and health records.
Qualifications:
- A bachelor’s degree in a human services field from an accredited college or university is required.
- A Valid Driver’s License and driving record that meets agency insurability requirements is required.
- Bilingual Skills (English/Spanish) desired
- Successful completion of background, fingerprinting, and drug testing
Experience:
- No prior experience is required, though it’s preferred.
Additional Eligibility Requirements:
- Annual Flu Vaccinations
- Annual TB Testing
