Women struggling with addiction have limited options for treatment and even fewer options if they are pregnant or have small children. The Wings Program provides residential and intensive outpatient treatment for pregnant women and their children aged birth to five. These comprehensive services facilitate recovery from addiction and promote family self-sufficiency and healthy lifestyle choices. Wings works closely with the North Range Behavioral Health Early Childhood and Family Connects programs, and continued treatment includes the children in a whole family approach.
North Range Behavioral Health believes no one should fight behavioral health challenges alone. As a Case Manager with Wings, you can expect to collaborate with clinical teams to secure and provide resources to clients with mild to severe mental health challenges and/or substance use disorders. Case Managers work directly with clients across the lifespan to provide compassionate and comprehensive bridge care while clients continue services. These services can include education and skill development to help clients move towards independence.
Key Responsibilities:
- Provide Support to Clients Work directly with clients as assigned by your supervisor. You may manage a caseload and provide services to help clients navigate their challenges.
- Help Clients Achieve Their Goals Assist clients in reaching their personal and treatment plan goals through effective interventions and support.
- Collaborate with a Team Work closely with therapists and other team members to coordinate care, monitor client progress, and help prevent crises or hospitalization.
- Connect Clients to Resources Guide clients in accessing essential services like vocational support, healthcare, housing, and financial assistance. Assist with applications and necessary documentation.
- Build Community Relationships Maintain strong partnerships with local organizations, landlords, businesses, and service providers to support client needs. You may also help manage contracts or leases.
- Facilitate Engagement & Education Encourage clients to participate in services and learning opportunities. You may lead psychoeducational groups, adjusting teaching methods to ensure meaningful participation.
- Teach Life Skills Support clients in developing daily living skills, such as managing finances, following medical recommendations, and practicing self-care.
- Provide Support to Families & Caregivers Offer guidance and education to caregivers, family members, and other key individuals to create a strong support system for the client.
- Adapt Services to Client Needs Meet clients where they are by providing outreach, home visits, appointment support, childcare assistance, and other accommodations to enhance engagement.
- Empower Clients to Thrive Help clients build coping skills, make informed choices, and develop problem-solving abilities to improve their well-being and independence.
- Track and Document Progress Monitor client progress toward goals and maintain accurate records using electronic systems. Ensure compliance with reporting requirements.
- Coordinate with Insurance & Funding Sources Work proactively with funding providers to align services with authorization requirements and ensure successful billing.
- Assess Client Needs When required, conduct assessments, develop treatment plans, and provide interventions to support clients behavioral health needs.
- Administer Screenings & Evaluations Facilitate assessments tailored to specific populations or environments, such as primary care or juvenile services.
- Advocate for Clients Work with courts, schools, and government agencies to advocate for client needs, ensure access to services, and provide representation in various settings.
- Coordinate Care with Healthcare Providers Collaborate with hospitals and treatment centers to ensure smooth client admissions and discharges.
- Contribute to Team Success Take on additional responsibilities as needed to support clients and strengthen program effectiveness.
Qualifications:
- A bachelors degree in a human services field from an accredited college or university is required.
- A Valid Drivers License and driving record that meets agency insurability requirements are required.
- Bilingual Skills (English/Spanish) desired
- Successful completion of background, fingerprinting, and drug testing
Experience:
No prior experience is required, though its preferred.
