About the Role: Crisis Response Team – Mental Health Clinician:
 

Our dynamic Crisis Response Team is vital in providing high-quality behavioral health services to those who need them most.

As a key member of our specialized team, you will conduct critical assessments and care for individuals and families in crisis across Weld County and a 12-county region in Northeast Colorado. This rewarding role includes responding to emergency walk-ins, crisis calls, and after-hours on-call situations. You'll have an opportunity to make an immediate impact by performing thorough evaluations, identifying the best care options in the least restrictive settings, and facilitating connections to suitable resources.

In this position, you'll also offer intensive care coordination for Weld County clients and support discharge planning, aiding individuals in successfully transitioning from higher levels of care back into their communities.

As a clinician on the Crisis Response Team, you'll play a vital role in delivering strength-based, person-centered care. You'll assist clients in creating individualized service plans, tracking treatment progress, and ensuring thorough clinical documentation. This role requires active collaboration with clients, community partners, and multidisciplinary teams to provide smooth, trauma-informed, and evidence-based care.

Key Responsibilities:

• Conduct assessments, goal planning, and treatment for mental health and/or substance use issues, utilizing required instruments and formats within appropriate timelines.
• Develop and update strength-based treatment plans, continuing care plans, and discharge summaries; make timely referrals.
• Observe clients, assess diagnoses and mental status, and implement treatment plans based on clients’ needs and functioning levels.
• Collaborate with clinicians, law enforcement, external providers, and psychiatrists to ensure comprehensive care.
• Work with clients on safety planning, symptom management, support system identification, and risk mitigation.
• Teach clients behavioral skills, coping strategies, and problem-solving techniques to improve well-being and daily functioning.
• Support evidence-based practices to ensure effective interventions within the program.
• Provide case management and refer clients to additional resources (e.g., vocational, medical, housing, residential treatment).
• Maintain accurate electronic health records, ensuring ethical coding and billing in compliance with agency standards.
   

Qualifications:

• Master’s degree in psychology, social work, or a related field.
• Valid driver’s license and a driving record that meets insurability requirements.
• Successful competition of background, fingerprinting, and drug testing
• Bilingual (English/Spanish) skills are a plus.

Experience:

• No prior experience is required, though it’s preferred.
