As a Case Manager at North Ridge Health and Rehab, your primary responsibility will be to serve as an advocate for our residents and their families. You will work closely with residents and their loved ones to ensure their physical, emotional and social needs are met. You will be responsible for documenting residents charts, conducting assessments and coordinating various aspects of care. Your role is crucial in promoting a positive and suportive environment for our residents.
Key Respnsibilities:
Strengths in the area of Social Services and Case managment. Prefer experience in long term and memory care of at least 1 year Must be able to work with multidisciplinary team and have excellent written and verbal communication skills. HMO, Medicare and Insurance experiance preferred. Must have knowledge of community resources and ensure compliance with all HIPAA regulations. Passionate about social services and case managment? WORK SCHEDULE
