**Job Title:** RN Case Manager - Hospice
Description:
We are seeking a compassionate and skilled Registered Nurse to join our team as a RN Case Manager in the hospice setting. As a RN Case Manager, you will be responsible for providing high-quality patient care and support to individuals and their families facing end-of-life issues.
Key Responsibilities:
- Plan, organize, develop, and direct the nursing care of patients in accordance with federal, state, and local standards, guidelines, and regulations.
- Assess and coordinate pain/symptom management programs with the participation of family and patient, attending physician, and Interdisciplinary Team members.
- Facilitate freedom of choice, relief of suffering, and death with dignity for patients facing end-of-life issues.
- Educate patients and families on disease processes and expectations related to disease progression.
- Provide support and education for families based on the hospice philosophy of care, which includes physical, psychological, spiritual, and financial comfort.
- Participate in Interdisciplinary Team meetings and update Plan of Care (POC) for assigned patients based on identified problems and needs.
- Ensure patient care is delivered at the appropriate level of care.
- Manage medication profiles to prevent polypharmacy and ensure optimal symptom management.
- Coordinate services, including communicating and making assignments for visiting nurses, home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services.
Requirements:
- Registered Nurse graduated from an accredited program currently licensed and in good standing with the Wisconsin State Board of Nursing.
- One to two years of experience in nursing in an acute care setting and 1 year experience in home health, hospice, or comparable experience preferred.
- Able to work cooperatively with other disciplines, including social workers, chaplains, CNA's, and volunteers.
