Registered Nurse Case Manager
Responsibilities:
- Provide comprehensive case management services to patients in various healthcare settings.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients.
- Conduct assessments to identify patient needs and develop appropriate interventions.
- Coordinate and facilitate communication between healthcare providers, patients, and their families.
- Monitor patient progress and adjust care plans as needed.
- Advocate for patients' rights and ensure they receive high-quality care.
- Educate patients and their families on disease management, treatment options, and available resources.
Requirements:
- Valid Registered Nurse (RN) license in the state of Texas.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team environment.
- Proficient in electronic medical record (EMR) systems. North Star Hospice uses Axxess charting system.
- Knowledge of case management principles and practices.
- Ability to prioritize tasks and manage time effectively.
