North Star Services, Inc. is a private, non-profit agency dedicated to providing high-quality, cost-effective support coordination services for people with intellectual disabilities of all ages in Central Pennsylvania. Our mission is to help people identify, reach, and maintain their personal goals through effective support coordination. As Blair County’s first and largest Support Coordination Organization, we are valued for our knowledge and experience in connecting people to community resources to help them reach their dreams.
We are seeking a Support Coordinator to serve Ebensburg, PA areas. This role is essential in advocating for the rights of adults ages 22 and up with developmental disabilities, helping them achieve personal goals by managing and monitoring services, facilitating support teams, and ensuring health safety. This role requires strong skills in social work, management, program development, and budgeting, along with a Bachelor's degree and experience in the field, to effectively empower individuals and oversee program initiatives.
Responsibilities:
- Coordinate and provide support services to individuals with developmental disabilities
- Develop and implement individualized service plans
- Conduct assessments to determine client needs and goals
- Collaborate with customers, families, and other professionals to develop and monitor treatment plans
- Provide ongoing case management and advocacy for customers
- Monitor customer progress and make necessary adjustments to service plans
- Maintain accurate and up-to-date records
- Ensure compliance with all applicable regulations and standards
Qualifications:
- Bachelor's degree in a relevant field (e.g. social work, psychology, counseling)
- Experience working with individuals with developmental disabilities
- Strong administrative and program management skills
- Excellent relationship management skills
- Knowledge of behavioral health principles and practices
- Experience in program development and implementation
- Ability to facilitate team meetings
- Proficient in budgeting and financial management
- Valid Driver’s License
