Job Summary:
We are seeking a dedicated and experienced Case Manager to join our dynamic team at North Star Wellness Center. In this crucial role, you will provide comprehensive support and guidance to inmates housed at Fresno County Jail. Your primary responsibilities will include assisting these individuals in accessing vital resources and services designed to facilitate their rehabilitation and foster successful reintegration into society upon their release.
The focus of our program is specifically on inmates aged 18 to 30 who have histories of gang involvement and are classified as medium to high risk for reoffending. Understanding the unique challenges faced by this demographic is essential, as your work will not only aim to prepare participants for life after incarceration but also to empower them with the tools and knowledge necessary to lead fulfilling, productive lives. As a Case Manager, you will be responsible for developing individualized case plans tailored to the needs of each participant. This includes conducting thorough assessments to identify barriers to reintegration and connecting inmates with educational and vocational training opportunities, mental health services, and substance abuse treatment programs.
You will also coordinate with community organizations, parole officers, and other stakeholders to ensure a seamless transition from jail to the community. Your role is vital in creating a supportive environment that encourages personal growth and accountability, with the overarching goal of reducing the likelihood of reoffending. Key Responsibilities:
· Assess the needs of inmates and develop individualized case plans.
· Provide counseling and guidance to inmates regularly.
· Connect inmates with resources and services, including substance abuse treatment, mental health services, education, and job training programs.
· Monitor inmate progress and adjust care plans as needed.
· Maintain accurate and detailed records of inmate interactions and progress.
· Participate in multidisciplinary teams to address inmate needs and develop strategies for successful reentry.
Requirements:
· Bachelor’s degree in social work, Counseling, or related field.
· Two years of experience in case management or a related field.
· Knowledge of the criminal justice system and the challenges incarcerated individuals face.
· Excellent communication and interpersonal skills.
· Ability to work effectively with diverse populations.
· Strong organizational and time management skills.
· Bilingual or multilingual skills.
Knowledge, Skills, and Abilities:
· Ability and skill to deliver psychoeducational material and facilitate groups.
· Knowledge of virtual meeting platforms.
· Ability to evaluate client risk for future violence.
· Knowledge of Microsoft Windows applications
· Ability to write reports and business correspondence.
· Ability to provide exceptional customer service and communication skills, both written and verbally.
· Ability to effectively present information and respond to group, manager, and client questions.
· Ability to understand the needs of offenders and survivors of domestic violence, child abuse, or other forms of trauma and be willing to address the needs of the clients with actions designed to create change.
· Be sensitive to issues of family and community violence, particularly security issues.
· Ability to work competently and without discrimination toward diverse populations, cultures, races/ethnicities, socioeconomic positions, ages, religions, physical/mental positions, and sexual orientations.
