North Suffolk is looking for a Care Coordinator to join our Mobile Crisis Intervention (MCI) Team! The Care Coordinator is part of an emergency services team providing continuity of care to all clients, children and families and allied services. This position requires coordination of Emergency Service / MCI and Call Center workflows, facilitating and documenting service referrals and documentation of client information. This position requires some direct contact with clients, providers and other community partners and stakeholders.
This is an on-site position within the MCI Call Center, 40 and 30 hours per week positions available.
Language Differential : $5,000 for Spanish and Portuguese speaking candidates strongly preferred!
What You’ll Do :
- Assist call center clinicians in responding to both non-crisis and crisis calls. May assist in taking triages from hospitals for continued follow-up in the community by MCI. May outreach to clients to coordinate evaluations discussed earlier in the day but scheduled for a later time.
- Outreach to MCI clients to complete safety plans, as needed
- Assist in the coordination and completion of scheduling urgent psychopharm appointments at NSCS, referrals for community-based services and higher levels of psychiatric care
- Provide site-based administrative responsibilities in collaboration with program managers (ex. Keeping bed search and other resource lists up to date, closing charts etc.)
- Attend regularly scheduled staff meetings and supervisions
- Weekends, Holiday and shift rotation will be required.

- Education :
Bachelor’s degree in relevant mental health field
Minimum one year in a psychiatric emergency service or similar setting required
