North Suffolk Community Services (NSCS) is seeking a Master’s Level Clinical Case Manager to join our Latino Community Advance Response Team (CART)! The Latino CART is a bilingual clinical intensive case management team that works exclusively with uninsured and underinsured Hispanic individuals and their families living in Chelsea, MA and the surrounding communities. Within this 2-person team, the Latino CART Clinical Case Manager provides clinical services including supporting the person-served find solutions to problematic behaviors via the use of evidence based practices and skill building as well as facilitating linkages with other community based services and natural support systems to support the person-serve thrives within their community.
This is a Full-Time position; the schedule is Monday - Friday 9am-5pm.
What you’ll do:
- Provide clinical services to qualifying individuals in their homes and/or the community.
- Provide person centered individual therapy to address mental health needs that prevent stability within their community.
- In conjunction with the Latino CART Navigator, assist qualifying clients in navigating and connecting with community resources that will further support the person’s stability within their community. (SNAP, Housing, Transportation, Legal services, etc.)
- Collaborate with community network alliances to establish a community safety net to further support aftercare services.
- Maintain regular weekly contact (face-to-face and telephone) with all qualifying Latino CART clients.
- Other duties as assigned.
What you’ll need:

Education:
- The position requires a Master’s degree in the mental health field (including but not restricted to, counseling, family therapy, social work, psychology, etc.)

Skills:
- Must be able to communicate effectively in Spanish, please specify language in resume.
- Must have a valid MA driver’s license and reliable transportation for off-site travel, required.
