North Suffolk's Community Behavioral Health Center (CBHC) program is looking for an In-Home Clinician to join our team! The CBHC In-Home Clinician provides the administration of the agencies’ CBHC programming, including by providing community/clinic-based services, primarily to older adults.
This a Full-Time position; 40 hours per week. Pay differential for bilingual Spanish or Portuguese clinicians!

North Suffolk's Community Behavioral Health Center (CBHC) program is looking for an In-Home Clinician to join our team! The CBHC In-Home Clinician provides the administration of the agencies’ CBHC programming, including by providing community/clinic-based services, primarily to older adults.
This a Full-Time position; 40 hours per week. Pay differential for bilingual Spanish or Portuguese clinicians!
What you'll do:
- Provides in-home individual therapy for identified caseload of elder clients
- Conducts intake assessments for clients of all ages, which include diagnostic evaluation, treatment recommendation, and referral.
- Completes all clinical documentation in a timely manner, and in accordance with medical necessity, agency, and payer guidelines.
- Provides weekday evening coverage (until 8pm) on a rotating basis, as requested by the Director of Outpatient Services or designee.
- Provides weekend coverage for CBHC clinics (Saturdays and/or Sundays, 9am-5pm) on a rotating basis, as requested by the Director of Outpatient Services or designee.
- Additional duties as assigned by the Director of Outpatient Services or designee
- Education/Skills:
- Masters-level degree in Social Work, Psychology, Counseling, or related field with commensurate licensure
- Valid independent clinical license (LICSW/LMHC) preferred
- Certificate, specialization, and/or training relating to serving elders is preferred
- Fluency in Spanish or Portuguese preferred
- Experience:
- Experience providing in-home or clinic-based therapeutic services to older adults with a variety of diagnoses and presentations
