What are the responsibilities and job description for the Behavioral Health Case Manager position at North Texas Area Community Health Centers Inc?
1) Carries a case management caseload and will help patients locate and maintain through a social determinants of health lens; medical, financial assistance, behavioral health, dental services, vocational, housing, educational resources, socialization opportunities and transportation, to support patients thrive successfully in the community.
2) Validates prior authorization if case requires approval prior to service post initial comprehensive visit.
3) Develop Individual Service Plan (ISP) for patients in caseload to insure integrative care, coordination, and monitoring of all aspects of services for each patient.
4) Ensures that the primary care provider team inputs are coordinated with ISP development according to established Texas Department of State Health Services (DSHS) Clinical Management Behavioral Health System (CMBHS) criteria and timeline.
5) Support patient service flow through the maintenance of collaborative processes with various North Texas Area Community Health Centers (NTACHC) staff to enhance the “patient experience”.
6) Works with primary care clinical team and leadership in ensuring patient flow for behavioral care health services, including scheduling of patients.
7) If necessary, provides outreach and crisis intervention to patients to assess behavioral health needs and insure proper service intervention.
8) Maintains and initiates extensive liaison work with community agencies including behavioral health services, specialty services, financial support, welfare, court, social security, and housing to maximize the quality of life for patients.
9) Collects standardized patient related data for research, evaluation, or administrative purpose.
10) Maintains patients’ records with up-to-date patient information, progress notes and ISP, to insure coordination and adequate treatment services.
11) Visits patients in their homes or residential settings and monitors the safety of the environment.
12) Facilitates referral of patients to programs best suited to their service’s needs. Coordinates and follows-up on transitions to various community programs.
13) Safeguards human rights of patients through familiarization with providers and domestic environment, support and advocacy and reporting of human rights.
14) Performs other related duties as assigned.
