Job Summary and Qualifications
As a Manager of Nurse Navigation, you will oversee the nurse navigators that execute patient navigation and support the strategic goals of the division/market – providing leadership support to all clinical resources for navigation. You will have the primary responsibility to develop, manage, and mentor the nurse navigators while serving as an advocate and educator for patients throughout the continuum of care. You will also serve as a direct leader for local and remote nurse navigators to execute mid and long-range strategic initiatives to optimize operational performance.
What you will do in this role:
- Ensures navigators have the availability and operation of necessary equipment and/or resources to navigate patients effectively through the continuum of care
- Performs 1:1 rounding with navigators to provide feedback on performance and nurture growth; provides appropriate coaching as needed
- Implements corrective action and conducts performance evaluations
- Actively participates in the hiring and onboarding of navigators
- Assists in the development and follow-through of the division navigation team
- Drives process improvement and identifies opportunities for workflow efficiency and standardization. Ensures reporting is in place to demonstrate program outcomes and support performance improvement activities
- Provides leadership, direction, and guidance to assigned staff
- Coordinates and maintains the required training and skills of staff
- Effectively addresses colleague issues to promote a productive and healthy work environment
- Works collaboratively in concert with the Director of Care Navigation to ensure alignment with strategic goals, data interpretation, report generation, and facility-level planning within the division, corporate, and respective service lines
- Ensures standardization of navigation workflow and identifies opportunities in collaboration with stakeholders for improving patient care and outcomes
- Promotes research (as applicable) and integrates evidenced-based knowledge into the identification of best practices in the delivery of patient navigation
- Knowledge of regulatory agency standards and customer service expectations for the practice of patient navigation
- Responsible for operating within an approved budget
- Ensures appropriate productivity of navigators (e.g. monitoring caseload, timeliness, acuity of patient/complexity)
- Collaborates with corporate and the respective service lines to ensure an adequate knowledge of goals and program specifics
What you will need in this role:
- At least five years of experience in a healthcare field is required
- Three years of experience as a nurse navigator and/or patient educator is preferred
- At least one year of experience in leadership is preferred
Education
- Bachelor’s Degree in Nursing required
- Master’s Degree in Nursing preferred
Licenses and Certifications
- Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation.
