DESCRIPTION :
- Provide professional social work methods and theory to the diagnosis, prevention, and treatment of psychosocial dysfunction, impairment, or disability, including mental, emotional, and behavioral disorders of children, adolescents and adults.
- Provide mental health screenings, and initial assessments to medically-referred and other patients (children, adolescents, and adults) in person or via telemedicine visit.
- Provides clinical therapeutic services to patients (children, adolescents, adults) including individual therapy and coordination of treatment.
- Makes appropriate adjustments in assessments, plans, and interventions based on the specific age, cultural, and religious needs of patients served.
- Provides both routine consultation and emergency assistance in crisis situations.
- Assumes responsibility for remaining current in job-related knowledge and skills and continuing education and licensure requirements.
- Coordinate and collaborate with the primary care providers to improve health outcomes.
- Facilitate referrals to other services (e.g., substance abuse treatment, specialty care, and community resources) as needed.
- Maintain accurate and timely records of activities, case management notes, and services provided to each patient.
- Consistently communicates in a professional and courteous manner.
