Job Summary
We are seeking a dedicated and skilled Triage Phone Nurse to join our healthcare team. The Triage Phone Nurse will play a crucial role in assessing patients’ needs, prioritizing care based on medical urgency, and coordinating treatment plans. This position requires a seasoned LVN, strong clinical judgment, excellent communication skills, and the ability to work efficiently in a fast-paced environment.

Duties

• Conduct initial assessments of patients to determine the severity of their condition.
• Prioritize patient care based on clinical urgency and available resources.
• Collaborate with physicians and other healthcare professionals to develop and implement care plans.
• Provide education and support to patients regarding their conditions and treatment options.
• Maintain accurate medical records and documentation in compliance with healthcare regulations.
• Assist in case management for patients requiring ongoing care or follow-up.

Requirements

• Current LVN license in the state of practice.
• Experience in case management and medication administration is preferred.
• Strong critical thinking skills and the ability to make quick decisions under pressure.
• Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.
• Knowledge of HIPAA and guidelines.
• Will need to be self starter and ability to stay on task.
• OB-GYN knowledge a plus but not required

Job Type: Full-time

Pay: $24.00 - $26.00 per hour

Expected hours: 40 per week
