What are the responsibilities and job description for the Hospice Patient Care Coordinator position at North Woods Home Care & Hospice?
QUALIFICATIONS:
1. Graduate of an accredited school of nursing
2. Current licensure as registered nurse with State of Michigan
3. Graduate from an accredited college with an Associates, Bachelor of Science or Master of Science degree in health care administration, public health nursing, or nursing preferred.
4. Minimum of 1 year training in home health care, health services, or related field, demonstrating leadership expertise. Hospice certification preferred.
5. Excellent verbal and written communication skills
6. Excellent administrative, organizational and leadership skills
PHYSICAL REQUIREMENTS:
1. Visual/hearing ability sufficient to comprehend written/verbal communication
2. Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing
3. Ability to deal effectively with stress
SUPERVISION: Evaluated and supervised by the Administrator
DUTIES AND RESPONSIBILITIES:
1. Participates in IDG.
2. Assists the Administrator with program planning, development, administration and evaluation of the Hospice program.
3. Reviews initial assessments of patients/families, and the Plan of Care developed for them, with IDG
4. Coordinates implementation of the Plan of Care for each Hospice patient with IDG.
5. Assists the Administrator with facilitating care between primary nurse, the IDG team and primary physician.
6. Attends bi-weekly IDG meetings, assuring all patients conditions are accurately reported.
7. Clinical record reviews of Hospice patients, written and/or verbal instructions, and observation of patient care when requested.
8. Assist Administrator as needed in employee trainings regarding hospice services and care.
9. Assess patient/family response to care.
10. Assist with Grief Support groups or at public speaking events as needed.
11. Understands and adheres to the HIPAA privacy and security practices of the agency and understands that failure to do so will result in disciplinary action possibly including sanctions.
a. Ensures that the privacy practices of the agency that are designed to protect the privacy, use and disclosure of protected health information are clearly understood and enforced including the agency’s Notice of Privacy Practices in accordance with Federal requirements.
b. Ensures that the security practices of the agency that are designed to ensure the confidentiality, integrity and availability of all electronic protected health information that the agency creates, receives, maintains or transmits are adhered to. This includes protection against any reasonable anticipated threats or hazards to the security or integrity along with protection against any anticipated uses or disclosures that are not required or permitted by the privacy practices.
12. Is primarily responsible for the Conditions of Partition for hospice certification. Ensures compliance of hospice agency operations with federal, state, and local rules and regulations.
13. Maintains and enhances professional and managerial expertise through appropriate educational and organizational activities. Remains current with developments in community, industry and legislative areas and with this knowledge provides innovation in leadership.
14. Cooperates with other health-related agencies and organizations to increase agency services to the community and ensure coordination and utilization of community resources.
15. Realizes the importance of being a good member of the community. Works cooperatively with other agencies and organizations in the community. Provides community service as needed.
16. Performs additional performance responsibilities as assigned and as necessary.
KNOWLEDGE AND ABILITIES:
1. Knowledge of agency's Hospice policies, procedures and programs
2. Awareness of community health needs in this area including gaps in service and ability to cooperate with community agencies
3. Knowledge of field staff concerns related to Hospice care
4. Ability to plan, coordinate, and guide case review
5. Ability to identify Hospice in-service needs of all levels of staff
6. Knowledge of Hospice philosophy of care
7. Ability to work side by side with State Surveyors, knowing the COPs
8. Ability to communicate with Hospice patients and families.
9. Ability to actively listen with Hospice patients.
10. Knowledge of current standards of practice for nursing in Hospice.
11. Ability to keep current with nursing skills and knowledge, especially concerning the Hospice philosophy.
12. Ability to work as a team member.
