What are the responsibilities and job description for the Population Health Clinical Diabetic Educator NBMC position at NorthBay Health?
At NorthBay Health, the Population Health Clinical Dietitian – Diabetic Educator, participates in planning nutrition components in Population Health. The Clinical Dietitian is responsible for and has the authority to deliver nutrition care according to the philosophy, policies, and goals/objectives of the Population Health Program. The Clinical Dietitian assesses, plans, implements, and evaluates nutritional processes for individual patient care. The Clinical Dietitian makes determinations regarding the progress of the patient toward nutrition goals and utilizes a professional relationship with the patient and family to facilitate those goals. The Clinical Dietitian modifies and delivers care specific to the behaviors/needs of the pediatric, adult, and geriatric patient with in the scope of service of the Population Health Program.
With appropriate certification as a Certified Diabetic Educator (CDE) this position would be qualified to support the Population Health Program, which would include being responsible for providing direct diabetic education, insulin teaching and management as well as insulin initiation and adjustment based on department protocols and California Diabetes guidelines for care.

Education: B.S. degree in Food and Nutrition or Dietetics with completion of one year internship or equivalent accreditation program.
Licensure: Currently Registered Dietitian by the Commission on Dietetic Registration. Certification as a Diabetes Educator required.
Experience: At least two years of experience in outpatient nutrition. At least one year of experience with population health programs preferred.
Skills: Charting and assessment skills. Organization, time management skills, and ability to analyze data; presentation and teaching skills.

- Excellent communication and interpersonal skills in dealing with the public, co-workers, patients, and families.
- Ability to remain flexible and positive within a rapidly changing patient care environment.

Hours of Work: Variable prescheduled according to business need.
Compensation: $51.05 to $62.07 based on years of experience doing the duties of the job.
