Northeast Counseling Services offers a supportive work environment where you can grow professionally while making a real difference in the lives of others. As a Care Coordinator, you will play a crucial role in ensuring children through transitional-age young adults with serious and persistent mental illness and others at risk of serious mental illness receive the care they need. Your primary responsibility will be to connect them with necessary resources such as medical, social, educational, and other services. To do this effectively, you will draw upon natural supports, community resources and specialized mental health treatment, rehabilitation and support services. Your ultimate goal will be to promote recovery and achieve specific outcomes of independence of living, vocational/educational participation, adequate social supports and reduced hospitalizations. This service will be delivered primarily in the family home and community. For this position, you will require a Bachelor's Degree in social work, psychology or a related field from an accredited college or university. In addition, knowledge of the mental health, drug and alcohol and social service field is expected. Strong organizational skills, good judgment, positive communication skills and self-motivation are also key requirements. Creativity and a proactive approach to seeking services for consumers are highly valued.
