Northeast Counseling Services is a leading provider of mental health services, and we are seeking a qualified Care Coordinator to join our team.
The successful candidate will provide assistance to children through transitional-age young adults with serious and persistent mental illness and others at risk of serious mental illness. They will help these individuals and their families gain access to needed resources such as medical, social, educational, and other services through natural supports, community resources and specialized mental health treatment, rehabilitation and support services. The primary goal of this position is to promote recovery and achieve specific outcomes of independence of living, vocational/educational participation, adequate social supports and reduced hospitalizations. This service is delivered primarily in the family home and community.
The ideal candidate will possess a Bachelor's Degree in social work, psychology or a related field from an accredited college or university. They should have knowledge of the mental health, drug and alcohol and social service field. Good judgment, positive communication skills, and the ability to organize their time to meet consumer needs in a flexible, non-traditional time frame are essential. Self-motivation, creativity, and aggression in seeking services for consumers are also required.
