Summary: This position may work remotely, depending on business needs. Under the supervision of the Direct Programs Manager, the VDC Person-Centered Counselor is responsible for providing the management, supervisory services, and administrative support necessary to coordinate, implement, and administer the activities of the VDC program between ElderSource, the U.S. Department of Veterans Affairs (VA), eligible veteran beneficiaries (and/or their representatives), and the fiscal staff. The Person-Centered Counselor provides technical assistance and serves as a consultant to veteran beneficiaries and their representatives.

I. Essential Duties and Responsibilities include the following:

1. Provides detailed VDC program orientation to veteran and/or representative including how the program relates to self-direction, plan creation, service provision, and budget management to allow the veteran to live independently in the community; also trains the veteran or representative in self-direction with the goal of empowering the veteran or representative to take more control over time.

2. Conducts home-based visits with veteran and/or representative to perform assessment, develop service plan, identify functioning level, sources of support, etc. and conducts reassessments when there are changes in condition, program or situation; addresses other concerns as they arise.

3. Interact with fiscal staff regarding participant and employee set-up, budget, and timesheet submissions.

4. Works with local VA Medical Center liaisons and ElderSource’s fiscal staff to identify, evaluate, and transition veterans into the VDC program; coordinate timely enrollment of referred veteran beneficiaries; prepare and distribute ongoing communication of issues; and work to resolve issues which may arise.

5. Maintains current listing of active veterans, caseloads, expenditures and budget spending plans, and ensures the Veteran Database is properly updated and maintained.

6. Participates in development of forms and procedures for program monitoring, and ensures timely completion and submission of all required program and monitoring documents.

7. Keeps current on issues and programs affecting veterans and acts as a resource for organizations and the general public.

II. Working Relationships
A. Fiscal: Upload documents into software platforms and communicate changes timely for Fiscal staff to process payroll and vendor payments timely and accurately.

B. Staff: Contribute to the workplace environment that is nurturing, supportive, safe, equitable, and welcoming.

C. Regulatory Bodies: Ensure compliance with all legal and regulatory requirements to ensure the Agency remains compliant, particularly with the VA.

D. Donors and Funders: Be responsive to program donors and funders by providing timely and accurate reports.

E. Program Participants:
a. Be responsive, courteous and professional when working with clients, ensuring they are treated with dignity and respect and receive the best quality service to help them live independently.

This position requires the employee to travel frequently throughout Northeast Florida, as directed. When company car is not used; cost of travel is paid by the traveler and will be reimbursed according to state travel reimbursement policies.

III. General Responsibilities:

1. Ensures that all client data is transmitted, stored, and disposed of according to the confidentiality requirements of ElderSource and all federal and state regulations.

2. Attends conferences, workshops, and meetings to keep abreast of developments and issues related to the functions of this position.

3. Accesses, inputs, and retrieves information from a computer.

4. Participates, during regular and irregular office hours, in ElderSource disaster operations performing disaster related functions as assigned by the ElderSource Coordinating Officer. The employee will determine his/her own availability for this function when (s)he has been directly affected by the disaster.

5. Performs other marginally essential duties, as directed.

V. Education and/or Experience
Bachelor’s degree (B.A./B.S.) from four-year college or university; or four years related experience and/or training; or equivalent combination of education and experience.

VI. Computer Skills
To perform this job successfully, an individual should have knowledge of Internet software; Spreadsheet software and Word Processing software.

Pre-employment background check and drug screen will be conducted on top candidate. It is mandatory that a candidate be able to pass a Level II criminal background check through the Department of Elder Affairs prior to starting work.

Salary: $18.06 / per hour
