Northeast Health Partners (NHP) is a mission-driven organization dedicated to delivering high-quality, integrated healthcare services to communities across northeastern Colorado. As the Regional Accountable Entity (RAE) for Region 2 under Health First Colorado (Medicaid), NHP oversees the coordination and delivery of physical and behavioral health services for Medicaid members. We ensure a full spectrum of high quality, whole-person care, promoting member education, preventive services, care coordination, and evidence-based treatments are core to our mission.

Position Summary:

The Practice Transformation Coach supports the Clinical Services, Operations, and Quality teams in their effort to build a culture of quality with regional physical and/or behavioral health practices. The Practice Transformation Coach will work across the region and with multiple clinics to support practices in improving operations, meeting performance metrics, and supporting practices along the payment continuum. The Practice Transformation Coach must be able to coordinate, interpret, and translate performance data, and critically review metrics; data should permit evaluation of opportunities to implement change and monitor ongoing results of clinical quality outcomes. Collaborate with NHP Leadership team on ongoing opportunities for improvement in Clinical Operations, Quality, Risk Reduction, Patient Outcomes; this may be in response to regulatory, payer, entity & health system requests as it relates to clinical quality.

Must reside in the State of Colorado or be willing to relocate prior to position start date.

Duties Performed:

  • Acts on a consistent basis to support the Mission, Vision and Values of NHP.
  • Support NHP goals, strategies, and mission as directed by the Leadership team/ Director of Quality
  • Primary administrative and project manager support for practice transformation using principles of PCMH and the NCQA application process, including managing overall timeline, facilitate scheduling of practice meetings, coordination of educational webinars, and formulation/dissemination of meeting minutes, assist with design of corresponding data reports, verification of application details, coordination and facilitation of uploading survey documents, etc.
  • Will serve as a key member of the NHP team to help transform the practices into allowing them to excel at providing excellent patient experience, and quality medical care. Will help practices to be well-positioned to meet requirements of Colorado Department of Health Care Policy & Financing (HCPF) programs.

  • Test and Referral tracking processes
  • Integration of continuous performance improvement activities.
  • Supports NHP to implement change required to achieve the clinical quality goals through development of routine feedback mechanisms, guidelines, etc.
  • Participate in practice improvement meetings, providing expertise, coaching and guidance as necessary.
  • Engage in problem-solving with practices to improve access and patient-centered care
  • Monitor and assess performance across different aspects of delivering patient-centered care
  • Encourage culture of continuous quality improvement in practice
  • Design and facilitate improvement projects to build foundational capabilities at a site in order to improve its performance
  • Serve as the primary liaison, teacher, coach, and facilitator for regional Primary Care practices
  • Perform practice assessments to provide both a baseline understanding and the basis on which to develop the practice redesign plan
  • Maintains tracking of project tasks and deadlines for clinics
  • Assist NHP leadership team with data design and presentation, as well as support front line staff with data translation, interpretation, and performance improvement efforts.
  • Provide project coordination and administrative support for key quality and performance improvement initiatives that require substantial interplay between practices and NHP
  • Consistently observes and reviews practice workflow processes for new ways of improving efficiencies, streamlining processing/preparation of reports.
  • Supports entity wide staff training and education efforts related to quality projects and performance improvement opportunities.
  • Participates in key meetings and completes other duties as assigned.
  • Ability to routinely travel throughout Colorado and Region 2.

KNOWLEDGE/SKILLS/ABILITIES

Education/Credentials:

  • Bachelor’s or master’s degree in public health, healthcare, or business administration, or another closely related field.
  • Lean Six Sigma Green Belt or Black Belt is a plus

Experience:

  • For Bachelor’s degree, applicant must have 5 years’ experience in areas identified below. Master’s degree must have 3 years.
  • Project or contract management of similar scope and size.
  • Experience with managed care insurance systems in behavioral and/or physical healthcare.
  • Experience with working on projects that serve Medicaid populations.
  • Excellent communication skills, approachable work style and a willingness to take on complex organizational and clinical project management activities.
  • Demonstrate strong/solid knowledge and skill with computers, spreadsheets and databases.
  • Proficiency in software applications that include, but are not limited to Microsoft Word, Excel, and PowerPoint.
  • Working knowledge of EMR and report development would be helpful, but not required.
  • Must be highly organized with the ability to work well with people.
  • Must have ability to work with highly sensitive and confidential information.

Knowledge of:

  • Performance improvement methodologies including, but not limited to Root Cause Analysis, PDSA cycles, A3, and/or the DMAIC model.
  • Federal, state and local regulations regarding delivery and documentation of healthcare services.
  • Organizational sensitivity regarding complex relationships with federal, state and local agencies, government entities.
  • Advanced computer programs, software applications, and data collection (e.g., Excel, Word, Outlook, project management software).
  • Advanced office management practices and operating procedures, including records maintenance.
  • Best practices for program development and enhancement, change management, and effective collaboration.
  • Project management principles, practices, techniques, and tools.
  • Applicable data privacy practices and laws.

Skills/Abilities to:

  • Ability to establish priorities, work independently, and proceed with objectives with limited supervision.
  • Work cooperatively and effectively with others.
  • Plan, prioritize, and manage multiple simultaneous projects and assignments with attention to detail to achieve goals within tight deadlines.
  • Communicate in a positive manner and interface effectively, both orally and in writing, with a wide range of individuals and constituencies.
  • Must have excellent written and verbal communication skills including, but not limited to, presentation skills.
  • Work with diverse individuals and groups on complex community issues and demonstrate professional and political aptitude.
  • Review and verify information for accuracy, manage and prioritize workload, maintain organized work environment.
  • Apply principles of logic or scientific thinking to a wide range of intellectual and practical problems.
  • Willingness to work flexible hours in order to meet the organization’s needs/demands.

Working Conditions:

Work is primarily performed in a standard office environment; however, a hybrid work model may be offered, blending in-office and remote work opportunities based on organizational needs and job requirements. Some meetings will occur at locations outside of the usual work location, including the use of conference and electronic meeting platforms (e.g., Zoom, GoToMeeting). All positions require travel within the counties NHP serves.

SALARY RANGE:

Salary Range: $45,0000.00 - $65,000.00. The base compensation represents the entry point of the range. Actual salaries will vary and may be above or below the range based on factors including but not limited to location, experience, education, licensure, and performance.
