The role involves coordinating and supporting participants' functional, clinical, and psychosocial needs, prioritizing independence and wellness.
Responsibilities include collaborating with a large, multidisciplinary team to develop person-centered goals, communicating changes in participants' conditions, and facilitating primary care and preventive services.
Compensation and Benefits: $160,000 - $175,000 plus Annual Bonus full benefits package.

Requirements
- Master’s degree in nursing from an accredited institution.
- Current licensure as a Nurse Practitioner in the state of New York.
- Minimum of 2 years of experience in family medicine or primary care settings.
- Strong clinical skills with the ability to manage a wide range of health conditions.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
- Proficiency in EHR systems.
- Bilingual in Chinese
