Responsibilities
- Work with CRT team to promote a safe and healthy living environment for consumers in their own homes, or CRT contracted or managed homes.
- Provide first aid, health monitoring and special care procedures as urgently needed; then refer to other medical/health providers.
- Implement and participate in the Individual Plan of Care/ Treatment Plan.
- Administer emergency medication as needed during scheduled hours.
- Educate and counsel clients, family members, and primary care providers on various medications, physical and psychological issues.
- Dispense over-the-counter, non-controlled and controlled medications per the written guidelines and under the supervision of a staff psychiatrist.
- Monitor and process relevant lab results (e.g., for Lithium level, e.g., Clozapine level), record results as required and inform NKHS prescribers (and sometimes PCPs as necessary).
- Provide nursing assessment and consultation as needed to support the Care Bed and CRT staff.
- Assist in coordinating healthcare follow up with healthcare providers for CRT clients with complex medical needs.
- Registered Nurse in Vermont, will consider Licensed Practical Nurse.
- Prefer at least three-year’s experience working in a mental health setting.
- Ability to effectively interface with a wide range of medical and professional clinicians.
