Northeast Kingdom Human Services in Vermont is seeking a Case Manager / Service Coordinator to provide Case Management and Service Coordination for adult clients with severe mental health needs in the Community Rehabilitation Treatment Program. As a Case Manager / Service Coordinator in a largely community based role, you will be in the lead working to facilitate the best services tailored to each individual's needs. ESSENTIAL FUNCTIONS
As the Case Manager / Service Coordinator daily activities will include making decisions and solving problems though analyzing information and evaluating results to choose the best solution. Provide regular observation and documentation of gathered information from clients, environment, colleagues and service providers. Effectively communicating and collaborating with supervisors and co-workers by telephone, in written form, e-mail, or in person. Case planning to establish long-range objectives and specifying the strategies and actions to achieve them. Actively work to build mutual trust, respect, and cooperation among team members, clients and service providers. Weekly supervision with supervisor to review client case's and work activities.
RESPONSIBILITIES & SCOPE
Provide participants with holistic Case Management services, including activities of daily living, social skills, employment, community integration, supportive counseling, linkages to care and coordination of services.
Develop individualized treatment plans that help participants meet their goals towards recovery
Positive working relationship with client and family.
Conduct safety assessments and provide crisis intervention when needed
Offer overall clinical care alongside an interdisciplinary treatment team
Develop community contacts and public awareness
Provide transportation to clients in a safe and properly insured vehicle in the performance of their duties.
Abilities:
Proficient in using a computer and Electronic Medical Records
Ability to work as a team member but also be able to complete independent work
Ability to remain calm and professional in high acute situations
Once a month on call/pager for weekend consultation as back up support to Emergency Services (Stipend).
QUALIFICATIONS
Bachelor's degree in human services or related field highly desired. High School Diploma with significant experience will be considered.
Knowledge of and experience with seriously and persistently mental ill adults.
Must have valid driver's license, motor vehicle and automobile liability insurance coverage ($100,000/$300,000)
SALARY & BENEFITS
Competitive Salary $20.00/hr plus, depending upon education and experience
