Licensed Practical Nurse (LPN), IDDS
Are you a Licensed Practical Nurse looking for a dynamic work experience backed by a supportive RN Team? As an LPN at NKHS you will be part of the growing DS Nursing team, providing training, oversight, consultation, nursing care, and program planning and development, in a variety of settings, to individuals served by the Developmental Services Division. Experience lots of variety and flexibility with a combination of office, community, and residential-based work. An opportunity for either a new LPN or seasoned professional.

RESPONSIBILITIES & SCOPE
Provide “nursing oversight” for non-licensed staff in residential and community settings, to include but not limited to physical assessment, monitoring and dispensing of medications, nursing care, provision of special care procedures, first aid, health monitoring, etc., for identified consumers.
Help train, oversee and monitor identified staff in medication administration for DS Consumers.
Provide physical assessments to individuals in the Developmental Services program and liaison with other NKHS services to address the needs of residents in various Agency settings.
Liaison with other community health service providers.
Develop individual service plans with consumer’s team for those consumers with identified health related needs. Consult with DS consumers, family members and staff on health and medical related issues.
QUALIFICATIONS
Vermont licensure as a LPN required
