Essential Duties and Functions
Direct Services:
Offer a range of therapeutic support, including individual, couples, and family therapy via in-person, phone, and telehealth sessions.
Conduct assessments and screenings to connect individuals with appropriate resources.
Care Coordination:
Coordinate care by engaging in regular meetings with embedded SASH staff and participating in monthly team meetings.
Outreach to primary care, psychiatric, and other community partners to ensure comprehensive support for individuals.
Education and Experience Requirements:
Master’s Degree in counseling, social work, psychology, or a closely related field.
Must be licensed or on the roster of Non-licensed Non-certified Psychotherapists.
Possession of a valid Driver’s license and access to a personal vehicle.
