Description
Description of duties: The Community Navigator will ensure seamless care coordination for individuals referred to and/or returning from a higher level of care such as hospitals, inpatient or residential settings. The Community Navigator will work as part of a specialty team to assist CCBHC enrolled individuals in access and linkage to needed community resources such as housing to ensure optimal engagement in the recovery process. This position will engage in aggressive outreach with individuals and provide transportation as needed.
Qualifications
Qualifications: Bachelor’s Degree in a behavioral health and/or related human services or must have completed the Certified Peer Specialist Training provided by an approved Office of Mental Health and Substance Abuse Vendor.
Supervisory Relationships: Reports to Community Navigation Supervisor
Skills and knowledge required: Life experience in navigating the behavioral health and other service systems preferred; Experience with providing care coordination in a health care and or human service field for individuals requiring access and linkage to needed resources. Must understand and have experience in navigating social service systems i.e., Physical health, education, employment, housing, insurance etc.
Amount of Travel/Special conditions: Frequent travel in the community to conduct client outreach. Transportation of clients in company vehicle as required.
Work Hours: 40 hours per week. Typically 9:00 am - 5:00 pm Monday - Friday. A flexible schedule will be required to meet the needs of clients and families.

- Ability to ascend and descend flights of stairs and to negotiate curbs.

Salary : $40,000 - $46,000
