SUMMARY:
The Youth in Transition Outcomes Specialist works to support the Case Manager in helping older youth achieve Single Case Plan goals and move towards safe, timely case closure during the transition to adult hood. This position involves working closely with all members of the assigned case management team; with older youth, biological families, and resource families; and with external agencies and partners. The Youth in Transition Outcomes Specialist will focus on placement and in-home cases.
Tasks will include direct work with families to address non-clinical supports, as well as work with external partners and systems to ensure that older youth have the support, they need in their communities to achieve reunification or other permanency.
Qualifications
MINIMUM QUALIFICATIONS:
Bachelor's degree in social work, social services, education, counseling, human services or related field required.
A minimum of 2 years of experience working with older youth, young adults & families preferred. Must have strong interpersonal skills and the ability to communicate well orally and in writing.
Must have a valid driver's license.

Salary : $42,000 - $43,000
