Position Overview
We are hiring a proactive and connected Outreach Coordinator to grow our referral base and build relationships with case managers, counties, social workers, and housing programs. This is not an entry-level position — you must have a proven track record of bringing in referrals and existing relationships in the HCBS or housing field.
Key Responsibilities
- Secure new referrals through existing relationships and ongoing outreach
- Promote Northeastern Home Group’s full range of services, including:
- Individualized Home Supports (IHS with/without training)
- Homemaker services
- Night Supervision
- Respite Care
- Adult Companion Services
- Housing Transition and Housing Sustaining Services
- Conduct regular outreach to case managers, county contacts, clinics, hospitals, shelters, and social service agencies
- Represent Northeastern Home Group at community events, provider fairs, and networking opportunities
- Collaborate with the intake team to ensure smooth follow-up on all referrals
- Track referral sources, outreach progress, and performance metrics
Required Qualifications
- Must have existing connections in the HCBS or Housing Stabilization field
- Proven ability to bring in referrals and build new client relationships
- Strong understanding of 245D Home and Community-Based Services
- Experience working with case managers, counties, or similar programs
- Excellent communication and relationship-building skills
- Must have reliable transportation and willingness to travel locally
- Multilingual skills are a plus (e.g., Somali, Amharic, Spanish)
