What are the responsibilities and job description for the RN Diabetes Educator position at Northeastern Tribal Health System?
Position Summary: The RN Diabetes Educator is responsible for providing education and support to patients referred to the Healthy Living Program for diabetes and pre-diabetes and is involved in programmatic activities that encourage the prevention of diabetes.
Position responsibilities:
- Assess the educational needs of patients.
- Provides person-centered, evidence-based diabetes and cardiometabolic education conducive to behavior change and improved quality of life across the lifespan.
- Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the patient
- Seeks, maintains, distributes and promotes appropriate educational materials.
- Advise and assist patients through the process of home blood glucose monitoring, injectables (medication/insulin), blood glucose pattern management, adjustment of insulin dose, or inter-current illness.
- Seeks to extend the IHS Diabetes Standards of Care to all patients of NTHS.
- Collaborates, advocates and confers with other members of the diabetes care team in developing person-centered diabetes plans.
- Contributes to the achievement of established department goals and objectives. Complies with ADCES Diabetes Education Accreditation requirements.
- Assists the Healthy Living Program Director with SDPI grant application, annual report, and maintenance of DSME registry.
- Engages in lifelong learning by annually completing 15 CEU in diabetes related topics, as required by the ADCES Diabetes Education Accreditation Program.
- Works towards gaining Certification as a Diabetes Care and Education Specialist.
- Other duties as assigned.
Essential skills and experience:
- Associates degree or Bachelor’s degree in Nursing.
- Knowledge of professional nursing principals and procedures.
- Two to Five years’ experience preferred.
- Excellent verbal and written communication skills
- Strong problem solving and critical thinking skills
- Broad background in the humanities, developmental theory, communicative skills, and the biological, psychological and social sciences.
- Understand and experience in applying learning theories for all age groups.
- Knowledge of cultural aspects within the Oklahoma Area and the ability to appreciate cultural differences.
- Knowledge of diabetes, risk factors, complications, treatments, and needed lifestyle changes.
Valued but not required skills and experience:
- Master’s Degree or higher.
- CDCES certification
