Behavioral Health Specialist – Danville Health Center
Danville Health Center, a part of Northern Counties Health Care, is seeking a dedicated and skilled Behavioral Health Specialist to join our integrated care team. We are considering both full-time and part-time applicants. This on-site role provides critical short-term counseling, crisis intervention, and behavioral health support within a primary care setting.
Key Responsibilities:
- Provide short-term counseling and support for patients with behavioral health needs.
- Conduct clinical assessments, establish diagnoses, and work with patients to set and track treatment goals.
- Offer crisis intervention services, both in person and by phone, including for patients managing chronic pain or addiction.
- Collaborate with primary care providers, psychiatrists, and community partners to coordinate care.
- Participate as an active member of the Community Health Team (CHT) and leadership/chronic care team.
- Provide behavioral health education to patients, families, and clinic staff.
- Assist in referring patients to community resources and external behavioral health providers.
- Maintain timely and accurate documentation in the Electronic Medical Record (EMR).
- Mentor clinical and non-clinical staff and contribute to quality improvement initiatives.
Position Qualifications:
- Master’s degree in Social Work preferred, but candidates with a master’s in a related license-eligible field will be considered.
- Licensed or license-eligible as a mental health provider in Vermont.
- Minimum of one year of clinical experience, ideally in a primary care or integrated care setting.
- Knowledge of DSM-5, behavioral health conditions, and crisis intervention strategies.
- Strong organizational, communication, and documentation skills.
- Ability to work independently and as part of a collaborative team.
- Proficiency in computer systems, including EMR use.
