Hospice Chaplain – Caledonia Home Health Care & Hospice
Caledonia Home Health Care & Hospice, a division of Northern Counties Health Care, is seeking a compassionate and dedicated Hospice Chaplain to provide spiritual support to patients, families, and caregivers. This position plays a vital role in the interdisciplinary team, ensuring that patients receive individualized spiritual care in alignment with their values and beliefs.
Essential Job Functions:
- Assess the spiritual needs of hospice patients and their families/caregivers.
- Provide direct, individualized spiritual care to patients and their families.
- Serve as a liaison and support to community chaplains and spiritual counselors.
- Attend patient care conferences and interdisciplinary group meetings as part of the care team.
- Offer consultation, education, and support to the interdisciplinary team on spiritual care.
- Develop personalized spiritual care plans, collaborating with local clergy as requested by patients.
Position Qualifications:
- Graduate of an accredited seminary or school of theology preferred (BD, MDiv, or equivalent theological degree).
- Experience working with death, dying, and grief counseling for individuals, families, and caregivers.
- Ability to work as part of an interdisciplinary team and contribute to holistic patient care.
- Comfort working in an interfaith setting, with respect for diverse spiritual beliefs and traditions.
- Ability to network with community clergy and congregations to support patient needs.
- Knowledge of and commitment to the hospice philosophy of care.
- Must be a licensed driver with an insured vehicle that meets state and organizational requirements.
