Field Nurses-RN/LPN

Northern Homecare and Community Home Health Care Services are looking for compassionate, skilled RNs and LPNs to join our team of home health providers. Our nurses enjoy an extremely flexible schedule, one-on-one patient care, and autonomy. If you have the necessary skills, experience and needed licensure, consider becoming part of our Home Health team in the fields! If you are an individual that’s looking for a full-time position with complete control over your schedule, or a nurse looking for a part-time position to supplement your current income, consider taking on home visits.

We have home visits available in Oceana County, Mason County, and Manistee County. What area are you interested in covering?

Requirements:
- Graduate of nursing school and currently holding a RN license/LPN.
- 1 year of previous nursing experience, preferred
- Previous home health care experience, preferred
- Ability to communicate effectively and work as a part of an interdisciplinary team
- Strong ability to use critical thinking and deduction when performing job tasks and evaluating patient needs
- Ability to consistently connect with patients one-on-one
- Responsible, compassionate, and caring demeanor

Job Responsibilities:
- Providing services in accordance with the patient’s Plan of Care
- Performing initial evaluations, building Plan of Care, and regularly reevaluating patient’s nursing needs
- Initiating appropriate preventative and rehabilitative nursing procedures
- Preparing clinical and progress notes for each patient visit via EMR
- Preparing summaries of care conferences on patients in a timely manner
- Informing Physician and Director of Clinical services of changes to patient’s condition and/or needs as appropriate
- Educating the patient and family/significant others in meeting nursing and related needs
- Completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition

Job Types: Start at Part-time/PRN with the ability to transition into full-time Pay: Based on experience / Negotiable
