The RN Case Manager is responsible for supervising, coordinating, monitoring, and following the plan of care for assigned patients. Case Manager provides nursing care, education and support to the patient/family and/or caregivers. You will also promote professional integrity, and participate in an interdisciplinary team.
Requirements:Responsibilities:
- Develop and implement nursing care plans.
- Facilitate coordination of physical, clinical, and psychosocial care among the interdisciplinary healthcare team and patient/family.
- Use technology to assess the health status and physical needs of the patient.
- Assess patients' knowledge and risk for complications.
- Set and reach realistic goals for patients.
- Provide education to patients and families regarding appropriate home care.
- Assist patients in arranging for needed home health services.
- Provide education to patients about their level of physical dependence on others for activities of daily living, daily routines, and when to seek out others for help.
- Provide education to caregivers about supportive care measures to incorporate into the home environment.
Experience:
- Minimum of 1 year nursing experience
License/Certification:
- RN License in the state of Illinois (Required)
