The Community Outreach and Disease Intervention Specialist will coordinate healthcare and disease prevention activities. Disease intervention activities include case management, resource coordination, provider and/or patient outreach, partner services and STD screening. They will assist the Disease Intervention Program and help coordinate community outreach and testing events in Northern Indiana.
Job Type: Full-time
Pay: $18.00 - $21.00 per hour
Expected hours: No more than 40 per week
