This unique role is ideal for a Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), or Social Worker (LSW) who is passionate about hands-on patient care and enjoys a dynamic, mobile work environment.
Position Overview
As a Care Navigator, you will play a key role in ensuring efficient and high-quality patient care. This position involves direct collaboration with our healthcare providers to enhance patient experience, streamline clinical operations, and support chronic disease management.
Key Responsibilities
- Encounter Preparation & Setup: Organize patient data, medical records, and equipment before visits.
- Patient Engagement: Assist in gathering health information, preparing patients for examinations, and ensuring comfort during encounters.
- Chronic Care Management: Assist with patient follow-ups, care coordination, education, and support.
- Data & Documentation: Maintain accurate and timely records in the electronic health record (EHR) system.
- Collaboration & Communication: Work closely with providers, patients, and facility staff to facilitate seamless healthcare delivery.
Qualifications & Requirements
- Must hold one of the following certifications/licenses:
- Certified Medical Assistant (CMA)
- Certified Nursing Assistant (CNA)
- Social Worker (LSW)
- Valid driver’s license with a clean driving record.
- Previous experience in healthcare settings, patient care, wound care, or chronic disease management is preferred.
- Strong interpersonal and communication skills to engage effectively with patients and care teams.
- Proficiency in electronic health records (EHR) and basic computer applications.
- Ability to work independently and adapt to a mobile healthcare environment.
