What are the responsibilities and job description for the COMMUNITY HEALTH WORKER ( EMBRACE PROGRAM) position at Northern Manhattan Perinatal Partnership?
Northern Manhattan Perinatal Partnership (NMPP) in collaboration with New York Presbyterian (NYP) seeks Community Health Workers for the Embrace Program.
The mission of NMPP is to assist women in the Central Harlem, East Harlem, and Washington Heights community have healthy birth outcomes.
The CHWs will work in the OB/GYN Department and the offices of NMPP . The CHW will empower the clients to improve their health outcomes by providing culturally sensitive education, peer based support and connections to community based support social services. The CHW will enroll pregnant and post partum women into case management services to address their social support needs and to ensure postpartum and well baby visits.
RESPONSIBILITIES
* Conduct home visits when necessary to assess the need for other services and also to provide health education sessions.
* CHW will work with clients to create a service plan to assist clients with their needs, as well as provide health education services
* Provide advocacy/ escort to clients to medical appointments. government agencies, as needed
* Connect clients to community resources when needed
* Assist clients with scheduling of medical appointments
* Work closely with service providers to ensure that clients are receiving a continuum of care, The services may include family planning, health insurance.
* Maintain up-to-date case records documentation and database reflecting client contacts and service plan goals
* Participate in interagency collaborative activities
* Attend all required staff meetings and trainings
* Perform other duties as necessary that are relevant to the program
QUALIFICATIONS
Bachelors degree in Community Health, Social Work, Human Services, Public Health, or any closely related field.
Two years experience in community health
Comfortable conducting home visits
Strong interpersonal and engagement skills
Ability to work collaboratively
Must be computer literate and have basic Microsoft Office skills
Strong verbal and written skills in both languages
Ability to work independently and with a team
Ability to find community resources
Four days in the office, one day remotely
Bilingual English/ Spanish
Bilingual English/ Arabic
