A highly skilled Primary Care Nurse Practitioner is sought to join our dynamic team, providing comprehensive patient care with focus on preventative medicine, chronic disease management and patient education. We are looking for an experienced clinician passionate about building strong patient relationships and delivering quality primary care in a collaborative environment.
Responsibilities:
- Provide primary care services to adult patients.
- Perform comprehensive physical examinations and assessments.
- Diagnose and treat acute and chronic illnesses.
- Prescribe medications and treatments.
- Order and interpret diagnostic tests.
- Collaborate with other healthcare professionals to develop and implement patient care plans.
- Maintain accurate and up-to-date medical records using electronic health record system eClinicalWorks.
- Adhere to established standards of care, including infection control protocols and sterile processing procedures.
- Participate in utilization management activities to ensure appropriate use of healthcare resources.
Requirements:
- Valid Nurse Practitioner license in the state of New York.
- Master's degree in Nursing from an accredited program.
- Certification as a Nurse Practitioner in the appropriate specialty area.
- Prior experience in primary care or a related field is preferred.
- Excellent clinical skills and knowledge of evidence-based practice guidelines.
- Strong communication and interpersonal skills to effectively interact with patients, families, and interdisciplinary healthcare teams.
- Ability to work independently and make sound clinical judgments.
- Proficiency in using electronic health record systems for documentation and patient management.
