What are the responsibilities and job description for the Case Manager- Hope Springs position at Northern Nevada HOPES?
Looking for a Very Special Case Manager to Join our Team.
Case managers use a holistic, patient centered approach to identify barriers to health outcomes. Using evidence-based and harm reduction rooted practices, Case Managers provide care coordination in partnership with patients to meet their individual goals. Case Managers work with patients through completing assessments, care planning, service enrollment, and continued case review with the goal of improving patient self-sustainability and improved health. Case managers work with interdisciplinary teams and community resources to address identified barriers and vulnerabilities in support of the patents identified goals through care planning and continued support as needed. The Case Manager/patient relationship follows a cycle of trust building, assessment, referral, resourcing, and support in an effort to help patients actively participate in their goals of achieving better health outcomes. The case manager is responsible for upholding department and agency standards for patient care by engaging in case consultation, interdisciplinary meetings, and continued educational opportunities. Case Managers provide a myriad of services including care coordination that is population specific. Case manager workloads are assigned as singular or mixed population specific caseloads.
Hope Springs Case Management: Care Coordination, including providing ongoing services to program participants to identify and mitigate barriers to accessing resources, gaining employment, achieving sobriety and securing permanent housing. HSCM services are provided year-round 7 days a week for the residents of Hope Springs Tiny Homes, a 6-month bridge housing program.
