Requirements
Responsibilities:
- Evaluation of physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability.
- Provide clinical recommendations for therapy changes to the physician and facility staff as appropriate.
- Verify patient medical information to conduct Drug Utilization Review (DUR) on all prescriptions and review clinical edits to ensure there are no adverse drug interactions or allergies.
- Verification for completeness and accuracy of all new and refill labels and orders. Check for content and accuracy of all medication packaging and labeling including emergency drug kits
- Maintain resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including OTC and home remedies)
- Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances; assist in maintaining a perpetual inventory for schedule II drugs
- Work with appropriate pharmacy technician in reconciling Controlled Substances perpetual inventory as required by management for compliance with state and federal regulations
- Provide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customers
- Document all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record
- Provide evening, weekend, and holiday coverage during rotation and as required by management
- Follow all applicable government regulations including HIPAA
- Other duties as assigned; Job duties may vary depending on business needs
- On-call responsibility every 4th or 5th week on a rotation and as needed.
Minimum (Required) Qualifications: (must be met in order to perform the job at the required level)
- Bachelor’s degree, or PharmD from an accredited School of Pharmacy or Pharmacy Doctorate
- Current Pharmacist license active and in good standing in the State of Maryland
- Long-Term Care pharmacy experience preferred
- IV clinical experience preferred
- Ability to interact with other team members and provide support.
- Ability to work flexible hours, which may include evening, weekends, and holidays
Preferred Qualifications:
- Experience using QS/1 and DocuTrack software
- Experience using Microsoft Word, Excel and Microsoft Outlook
