POSITION SUMMARY : The primary goal of the NVIH Community Healthand Outreach Worker (CH / ORW) position is to identify individuals, families and communities'strengths and needs. The CH / ORW supports individuals, families and communitiesin developing and meeting health enhancement goals. This is done in part bypresenting resource options, providing health education and supportingdecisions that promote well-being. The CH / ORW is a health advocate for NativeAmerican and all NVIH patients in coordination with other related departmentsof Northern Valley Indian Health and community agencies. CH / ORW's receive training in a wide varietyof health topics to be able to provide basic health teaching and coaching. Their services are individual, family, andcommunity focused and related to all ages.
ESSENTIAL JOB FUNCTIONS : Essentialfunctions, as defined below may include any of the following knowledge, skills,and abilities. The list that follows isnot intended as a comprehensive list, but rather to provide a representativesummary of the major duties and responsibilities. Incumbent(s) will be required to perform avariety of duties listed, including but not limited to :
- Provide care coordination for any NVIH patient identified as at-risk per ACES scoring; coordinate with internal and external resources.
- Participate in the overall Community Health and OutreachProgram under the direction and guidance of the CommunityHealth and Outreach Director(s).
- Provide care coordination services to the servicearea by :
o Assistingpatients in identifying service needs and appropriate resources.
o Makingappropriate in-house or out-of-agency referrals.
o Assistingeligible patients in securing available financial assistance for health carerelated expenses.
