Join the NVFS Family : We are looking to hire a Bilingual -Dari speaking- Case Manager to join the NVFS Multicultural team (MC)! As the MC Case Manager, you will provide individual and group services that help promote the overall wellbeing of trauma survivors, conducts outreach and trainings in the community, serves as liaison with community partners, and in general, represent the case management program in the community. As the Case Manager, you will provide intensive social services case management to MC clients who are typically immigrants and refugees exposed to traumatic experiences including but not limited to domestic violence, sexual assault, trafficking, etc., to ensure their comprehensive needs are met, including social, medical, psychological, spiritual, and legal. As the Case Manager, you will assess client needs, shares resources, makes referrals, and meets regularly with active clients both virtually and in an office setting. As the Case Manager, you will work with a team that supports the goal of moving clients towards greater self-reliance. As the Case Manager, you will also serve as a liaison with other service providers on behalf of clients.

This job has a salary range within the mid-to-high 40s.
In this role, you will be responsible for :
Client Service Delivery :
- Conduct initial screening process, including documentation, to determine client's eligibility for services under specific grants or contracts.
- Manage caseload of approximately 70 client cases per year; manage development of a client-centered treatment and service plan based on information gathered during assessments and consultations.
- Conduct comprehensive case management assessment to determine individual client strengths, needs, resources and support systems.
- Work with clients to identify and prioritize their needs and establish goals for service.
- Serve as referral / liaison between clients and other internal and external treatment programs.
- Research and coordinate internal and external resources; link clients with community providers to provide collaborative services.
- Make referrals to connect clients to needed resources and services.
- Actively participate in training and case consultation sessions with treatment team including presentations of cases.
- Plan, develop, and facilitate client cultural orientation, workshops and education programs based on community need.
