Job Summary
The RN Care Coordinator triages patient care needs, provides clinical leadership to the clinic care team, coordinates patient care services, provides patient care and ensures timely access to care. Uses standardized, medical staff approved protocols to provide patient care, telephone triage, advice and health education for ancillary staff, clinic patients, and callers to the clinic. This position provides age specific and developmentally appropriate nursing care and triage for neonate, infant, pediatric, adolescent, adult and geriatric patients.
This is a temporary position expected to last no longer than 12 weeks.
Compensation
We are committed to providing wage ranges for all open positions. Please note that the specific compensation for this role will be determined based on experience, education and internal equity considerations. The hourly wage range for this position is: $35.78-$50.51. This range reflects the base salary for this position.
Key Accountabilities

Triages patient care phone calls and walk-ins to clinic.

Assess patients’ needs with triage procedure book on phone to determine appropriate level of care.

As appropriate, schedules appointments with providers.

Using SBAR format, document in EMT patient chart.

Triages walk-in patients for level of care needed.

Develops teaching plans for patients.

Performs duties as clinic RN as needed

Assesses and intervenes with appropriate care.

Provides accurate and timely documentation.

Provides safe and effective nursing care and support to provider team.

Delegates tasks to unlicensed care team as needed to implement care.

Provides clinical leadership to clinic team to ensure age-specific, developmentally and culturally appropriate care is provided to patients.

Ensures quality clinical service delivery at clinic.

Problem solves with/for clinic team, removing barriers and facilitating collaboration of care.

Participates in quality improvement work and development of protocols, procedures and policies. Provides health education to clinic team supporting growth and enhancing team performance.

Serves as a role model, upholding high standards and consistently demonstrating the organization’s values and excellent customer service.

Develops care plans and interventions, implements the plans and evaluates progress. Collects, prioritizes and synthesizes comprehensive information and data pertinent to patients’ health needs.

Coordinates patient care for assigned practice team.

Qualifications
-
Current and unrestricted licensure as a RN in the State of Minnesota.

Basic Life Support certification per American Heart Association guidelines within 6 months of hire (company paid)

3 years of clinical experience in a clinical/patient care environment

Clinical leadership experience

Specialty experience in ER or Urgent Care, preferred

Care coordination experience in a clinic environment, preferred

Experience working with electronic medical records program, preferred
